You must have a physical to practice, if you so not have a physical by first practice still come to get equipment and watch practice

*** You must have your physical and CYO forms to turn in to BOTH schools. We need these on record at St. Therese also.***

Saint Elizabeth has a separate Emergency contact that need to be filled out.
